Cardiac tamponade 🩸 is a medical emergency where fluid (usually blood or exudate) accumulates rapidly in the pericardial sac, compressing the heart chambers, especially the right atrium and ventricle, impairing diastolic filling and reducing cardiac output. This can result from trauma, malignancy, pericarditis, uremia, or post-MI complications like ventricular wall rupture. Classic presentation includes Beck’s triad: hypotension, muffled heart sounds, and raised JVP. Additional signs include pulsus paradoxus (greater than10 mmHg drop in systolic BP during inspiration), tachycardia, and dyspnea. On ECG, look for electrical alternans (varying QRS amplitude due to swinging heart) and low voltage QRS. Chest X-ray may show a water bottle–shaped cardiac silhouette. Echocardiography is the gold standard for diagnosis, revealing pericardial effusion and diastolic collapse of cardiac chambers. Emergency treatment is pericardiocentesis, preferably guided by echocardiography, to relieve pressure. If untreated, tamponade can lead to shock, cardiac arrest, and death. Every medico must be alert to this high-yield, rapid-onset killer in both clinical practice and exams. 🚨🩺
